SIGNS OF BLOOD LOSS AND SHOCK

Signs and symptoms of shock;

Confusion and/or restlessness

Increased respiratory rate (>20)

Increase in pulse rate (rapid and weak)

Pale, cold clammy skin

Later stages;

Blood pressure drops and radial pulse may be lost

Breathing becomes laboured and weak - patient may be gasping for breath

Conscious level decreases causing confusion, disorientation, unconsciousness and death

CONTROL OF INTERNAL BLEEDING

Patients who have signs and symptoms of shock but no apparent external blood loss could be suffering serious internal bleeding. This can often be due to blunt trauma such as road traffic accidents or a fall from height.

Serious internal bleeding may occur in;

The chest

The pelvis

The abdomen

Femur fractures

Patients suffering major internal bleeding require immediate transfer to definitive medical care (hospital and surgery). Apart from the symptoms of shock the patient may also suffer bruising, swelling, pain and tenderness around the injured area.

TREATMENT

Manage the airway and breathing and administer oxygen (if available).

Control any external bleeding.

If possible (no pelvis or spinal injuries) raise the legs to 45o – Once the legs are elevated they must remain elevated.

Encourage the patient to lie still.

Try to keep the patient warm.

Continue to monitor the patient and be prepared to perform CPR.
